Dear Parents and Guardians:

On Friday, March 1, 2024, during students’ Impact classes, students will have the option to participate in the Indiana Youth Survey. The survey is funded by the Indiana Division of Mental Health and Addiction and is free of charge to any Indiana school that wishes to participate. This self-reporting survey asks students in grades 6-12 a variety of questions about substance abuse, mental health, gambling, and potential risk and protective factors for these behaviors. This survey is conducted for two purposes: to provide local schools and their communities with valid estimates of the prevalence of alcohol, tobacco, and other drug use among their students, as well as mental health and levels of risk and protective factors present in students’ lives; additionally, the survey will provide the state of Indiana with similar estimates of prevalence on a statewide basis. The results of the survey are confidential and in no way identifiable to your student in order to provide complete anonymity. 

A link to the survey and its contents will be made available on the LIS website. Participation in the survey is optional. If you prefer that your student not complete the survey, please contact the main office at 574-753-3797 by February 28, 2024.
